THE GAF SCORE THE MOST IMPORTANT PART MUST EXIST ON A PSYCHIATRIC MEDICAL DOCUMENT

JANUARY 2025.  Not complete to be edited. Before  my approach to The RCMP and or CSIS... Repeats a bit.

Above was my DECEMBER 2019 DISCOVERY. A STORY CHANGER on IDENTITY FRAUD.  This after a long searched for PHOTO ID of UBC Forensic Psychiatrist Dr. Elizabeth Zoffmann. If this is what Dr. Elizabeth Zoffmann looked like between 2016 and 2019 I do NOT believe that I have EVER MET HER. Was I being GRILLED by The CUPE National President to see what I knew about UBC FINANCIAL IRREGULARITIES, of which on that day I KNEW NOTHING. 

On the day of May 25,2001 where 7 1/2 YEARS Later it was written up as A Forensic Psychiatric Medical / Criminal Report which became my Life Long FORENSIC Psychiatric Medical/Criminal Record which STANDS today going into 2025.

In a sworn AFFADAVIT OPIC File NO. FO8-34272 dated August 31, 2009 by UBC Forensic Psychiatrist Dr. Elizabeth Zoffmann she swears that she was contacted by way of letter dated Friday May 18, 2001 by the Clients Counsel to perform an assessment on ME. Within the AFFADAVIT, NO WITNESSES  or Legal support was permitted by her Client in my DEFENSE. 

This would mean I would have NO KNOWLEDGE of this Evaluation happening until later than Friday May 18, 2001. With Monday May 21, 2001 being a Statutory Holiday the earliest I could possibly have knowledge of this would be the late afternoon or evening of Wednesday May 23, 2001. The Assessment was Scheduled and carried out from 9am on Friday May 25, 2001, leaving ME 1 1/2 days to prepare my side of the story. However she dates the Evaluation as being on Sunday March 25, 2001. Documents on a Mr. Steward who she talks about way more than ME who is Mr. Stewart. 

In her document where there is TWO differently FORMATTED Version's where on each it is  MOCKED that I presented in my defense a 22 Page Website. There was NO 22 Page Website. Even today in 2024 I have NO Knowledge on the ability of how to build a Website. There was 22 pages of detailed Point by Point. How could I possible put 22 Pages of DETAIL together in 1 or 1 1/2 DAYS. 

On  April 10 2001 I was given the Date May 25, 2001, the Place 601-1200 Burrard St Vancouver and the NAME of Elizabeth Zoffmann. How did I know about this which believed to be an Anger Management Consultation well over 1 Month before Dr. Elizabeth Zoffmann was approached by her Clients Counsel, which she documents as shortly before May 18, 2001 contacted by telephone.

This is connected to my MAIN BLOG where POINT BY POINT MAIN STORY ON ABUSE OF PSYCHIARTY This story where I couldn't get my head around FACTS that are very clearly documented. This from a May 25,2001 which I believed was some sort of Anger Management Consultation.  However turned out to be a FULL BLOWN FORENSIC PSYCHIATRIC MEDICAL / CRIMINAL EVALUATION. 

FACT ONE. That my CUPE National Union gave some sort of an ASSINGMENT to a UBC FORENSIC Psychiatrist Dr. Elizabeth Zoffmann to have ME Committed Under The Mental Health Act. 

FACT TWO. That the UBC FORENSIC Psychiatrist actually APPOLOGIZED for FAILING. As in "UNFORTUNATELY Mr. Stewart does NOT QUITE meet the criteria for certification under the mental health act ". So BORDERLINE INSANE. 

FACT THREE. That the UBC FORENSIC PSYCHIATRIC MEDICAL EXPERT suspected the cause of my very serious Mental Illness was what could have been an Aneurism, Cerebral Hemorrhage, Cancerous Tumor, Blood Clot or other potential killer. With this she RECOMMENDED that a full neurological examination including a CT Scan of the Head / Brain be carried out. 

FACT FOUR. The DATE MY Forensic Psychiatric Medical / Criminal Report was sent to The CUPE National / UBC Client was June 12,2001. To prevent worsening of my severe Mental Illness a RED FLAG 🚩 should have been raised.  Myself or even a family member should have been informed immediately, within 5 to 10 days of receipt by the Client again June 12, 2001.

FACT FIVE. The CLIENT as in CUPE National / UBC Hierarchy with Dr. Zoffmann made a COLLUDED DECISION, which was to HIDE this Forensic Report from myself and family and BLOCK this Life Determining Medical Procedure from being CARRIED OUT. 7 1/2 YEARS later on January 16,2009 was when the OFFICIAL REPORT WAS RELEASED to ME. 

FACT SIX. This if TRUE, the BLOCKING of this critical medical procedure by UBC and CUPE National Hierarchy with Dr. Zoffmann was to worsen my condition or END MY LIFE. 

I did NOT BELIEVE that The CUPE National gave an ASSINGMENT to Dr. Elizabeth Zoffmann to have ME Committed Under The Mental Health Act.  I did NOT BELIEVE that my CUPE Union at National Level with UBC HIERARCHY attempted to ... how would you put it... MURDER ME.

  

I BELIEVED IN FORGERY. 

The damage this has done cannot be forgiven.

1. It has totally destroyed my family life.

2. It was LAST USED against ME in June 2019 when I had to leave my employment as an Engineering Change Planner at a Vancouver Shipyard as I could NOT complete a required for Employment NATO Criminal Record Clearance. This was a much NEEDED employment, where I had to walk away in shame.

3. In early 2023 the HERIDITARY Factor of my documented Mental Illness was USED against a Family Member. (Privacy Respected at this point).

 

It can be USED on ME for LIFE, on my family Lineage for ETERNITY. 

Only I am capable of clearing this from myself an family lineage.

Who is Dr. Elizabeth Zoffmann?

I Googled and found this link. A MAN WHO BRUTALLY MURDERED HIS 3 YOUNG CHILDREN IS NO MORE DANGEROUS THAN THE MAN SITTING NEXT TO YOU IN THE PUB  This is Dr. Zoffmann.

On ME Dr. Zoffmann who for ONE RETALIATORY RUDE E-MAIL to my CUPE local 116 President there she was not SO KIND, not ONE good word for ME.

 

1. I very narrowly escaped being Committed under The Mental Health Act. Dr. Zoffmann APOLOGISED to her client for just failing. I was documented BORDERLINE INSANE. 

2. Dr. Zoffmann suspected a Brain Aneurism, Cerebral Hemorrhage, Cancerous Tumor, Blood Clot or something else going on in my head. She recommended a full Neurological Examination which included a CT SCAN OF THE HEAD / BRAIN. Myself or a family member should have been informed within 5 to 10 days of receipt by her CLIENT. I guess this was NOT considered the business of myself or family. The critical life determining medical procedure was HIDDEN and BLOCKED. 

2. In the last 3 pages it would be repeatedly documented that FIRE ARMS were yet to be found by COLATERAL MEANS. 

In my 50+ years in Canada, apart from my FORGED Psychiatric Medical / Criminal Record the closest thing I have to a CRIMINAL RECORD is ONE Photo Radar Ticket.

As per my MAIN STORY, due to continued oppression by my CUPE Local 116 Executive I wrote ONE Retaliatory RUDE E-Mail (March 13, 2001). On March 14, 2001 I Apologized with a Second E-Mail the next day. This is mentioned in her Forensic Psychiatric Medical / Criminal Report on ME. She also quotes NO Previous History of Violence.  So it was ALL ABOUT ONE RUDE E-MAIL written in retaliation to my CUPE Local 116 Executive. 

As per my MAIN STORY Three times I was humiliated and shamed in front of family friends and Co Workers as I was drummed out of the University to face Psychiatric Evaluations. These Evaluations were documented to continue well beyond 2010. My ONLY GIVEN OPTION was to END MY BELIEF that Dr. Zoffmann's Forensic Psychiatric Medical / Criminal Report on ME was a FORGERY and that I had been issued TWO Differently Formatted Versions. 

Below you will witness TWO differently Formatted Versions.

The First Version issued by the CUPE Nationals Dianne Jolly on December 06, 2008 and USED as TRUE to push a further Psychiatric Evaluation. This on February 2007 where NO Mental Illness was found. 

The Second Version issued by The CUPE Nationals Dianne Jolly on July 11, 2008. The EXACT same version I called the OFFICIAL Version issued by The UBC Freedom of Information Coordinator Christina Ulveteg on January 16, 2009.

From July 2008 the Second Version was USED as TRUE by UBC / HR'S Lindi Frost to push my THIRD Psychiatric Evaluation an alleged UBC Psychiatrist Dr. Alan Buchanan. This on June 04, 2009. Again NO serious Mental Illness was found. THE BUCHANAN REPORT    

Well lets say Dr. Alan Buchanan did find two BRAND NEW Factors of Mental Illness.

1. One was my BELIEF that Dr. Zoffmann's Forensic Psychiatric Medical / Criminal Report on ME was a FORGERY and my Belief in TWO differently Formatted Versions.

2. My BELIEF in a Large sum of money. I believe referring to this. A calculation financial or otherwise...correct or incorrect, is NOT a Factor of Mental Illness. If it is, well UBC better start looking at their MATHS Professors.

Anyhow Dr. Alan Buchannan did NOT have anywhere near the same level of Mental Illness as did Dr. Zoffmann did.  However I BELIEVED his report on ME was also tampered with, which falls under The Criminal Code of Canada. FORGERY SECTION 366 UBC / HR'S Lindi Frost the ONLY name on board. 

This is UBC Dr. Alan Buchanan's EXACT Diagnosis on ME , where NO Philadelphia Lawyer is required to spot tampering.   However a PASS on the coml

The above Diagnosis in June 2009 drew my attention to The Multiaxial System
and the GAF Score 

Dr. Elizabeth Zoffmann FAIL on completing he Differential Diagnosis. A TWICE FAIL. As she approved BOTH Versions. 

This led to the question of WHY the DIAGNOSIS was incomplete and The GAF Score did NOT EXIST in EITHER of the TWO Differently Formatted Versions of UBC Dr. Elizabeth Zoffmann's Forensic Psychiatric Medical / Criminal Report om ME (Below). This would be the FIRST THING that a cross examining Medical or Criminal Investigator would REVIEW. 

Two UBC Psychiatrists BOTH Clinical or Assistant Clinical Professors out of The UBC Department of Psychiatry would have to be Constant in there DIAGNOSIS.

The multiaxial system used in mental health diagnosis includes the following axes123:

1. Axis I: Mental health and substance use disorders.
2. Axis II: Personality disorders and mental retardation.
3. Axis III: General medical conditions.
4. Axis IV: Psychosocial and environmental problems.
5. Axis V: Assessment of overall functioning.

AXIS V The GAF Score  Would be the SOLE PURPOSE of a Psychiatric Evaluation more especially a FORENSIC Psychiatric Evaluation whereby the institutionalizing of a Canadian Citizen was the OBJECTIVE.   

This was my MEDICAL PROOF of FORGERY presented to UBC / HR'S Lindi Frost late September 2009.

This MEDICAL finding led ME to BELIEVE that both UBC and The CUPE National had BOTH issued FORGERIES. Also that The TRUE Document by UBC Dr. Elizabeth Zoffmann still REMAINED in the Archives of UBC or The CUPE National. Something more like an Anger Management type report.

However DID a Forensic Psychiatric Medical / Criminal Report OR Anger Management type report actually even EXIST. Moving on...  

PROVING THE FORGERY.

A FORENSIC Psychiatric Medical / Criminal Report where it is documented that a Canadian Citizen is being assessed for CERTIFICATION UNDER THE MENTAL HEALTH ACT has to be one of the most prestigious Court Regulated Medical / Criminal Reports in the land. This type of Report is filed under BOTH Canadian Medical and Criminal Jurisdictions.

Prior to this Extreme action to put a Canadian Citizen into an ASYLUM, it cannot be by ORDER of the Canadian Citizens National Union OR Employer. 

Prior to this extreme measure ONE Rude E-Mail combined with ONE Photo Radar would NOT Justify this.

This should have ENDED in the Summer of 2007

THIS FIRST VERSION WAS ISSUED BY THE CUPE NATIONAL'S DIANNE JOLLY ON DECEMBER 08, 2006. Used as TRUE by the CUPE National to push another Psychiatric Evaluation. This on February 2007.  A Neutral Psychiatrist who gave waring to UBC... "NO MENTAL ILLNESS, and leave this man alone".

By Canadian Law a Legal Medical document of any description CANNOT be ALTERED in any way and USED as TRUE. More so a FORENSIC Psychiatric Medical / Criminal Report. BLACKING OUT like this ACCEPTPABLE ACCEPTABLE but NOT EDITING OUT, or completely different FONTS AND FORMATING.

It is against the law to REMOVE Content from a Legal Document, more especially a Document that has been written up as a FORENSIC Psychiatric Medical / Criminal Report. If CONTENT can be EDITED IN... Likewise CONTENT can be EDITED OUT. This version was used as TRUE.

BELOW IS SECOND VERSION ISSUED BY THE CUPE NATIONAL ON JULY 11, 2008 , AND FROM THE UBC PRESIDENTS OFFICE ON JANUARY 16, 2009 USED AS 100% AUTHENTIC AND TRUE.

When I questioned the TWO Differently Formatted Versions. The CUPE National refused point blank to discuss this. They withdrew there representation leaving me in the hands of UBC / HR's Lindi Frost.  Frost was planning what was to be my Third Of Three Psychiatric Evaluations, with a UBC Psychiatrist of her choice "Dr. Alan Buchanan" UBC Clinical Professor. The only Medical Evidence to support the start of this CHARADE, by Lindi Frost was these two CUPE National Forensic Legal Medical Documents that were being used as TRUE MEDICAL EVIDENCE. On UBC / HR'S intervention UBC had NOT yet Released Zoffmann's Report.

The ASSINGMENT given by Frost for this Psychiatric Evaluation was to make ONE Version DISAPEAR. Dr. Alan Buchanan did threaten that MORE Evaluations were to follow if I continued to believe in two versions and in the Corruptions of the FORENSIC Medical Documents.

NOTE: in BOTH Documents the DIAGNOSIS is NOT as what is demanded Globally. BOTH CUPE VERSIONS were CORRUPTIONS...

THE SAME COPY ISSUED FROM THE UBC PRESIDENTS OFFICE. UBC FREEDOM OF INFORMATION COORDINATOR CHRISTINA ULVETEG ON JANUARY 16, 2009 AS 100% AUTHENTIC AND TRUE. ULVETEG AND UBC / HR'S KYLE CORMIER (WHO WAS ALWAYS BEHIND THE SCEENS ON MY CASE) WERE FIRED IN SECOND HALF OF 2009...COOINCIDENCE... I DONT THINK SO!

VERSION 2. After release by the CUPE Nationals Dianne Jolly on July 11, 2008, this version was taken by me in mid July to Doctor Robert Vroom at British Columbia College Of Physicians and Surgeons. Dr. Robert Vroom did NOT support the CUPE Nationals second release. His response was "it is not the College's Jurisdiction to investigate documents issued by your Union". Version 2 NOT supported by The British Columbia College Of Physicians and Surgeons. On January 16, 2009 Christina Ulveteg would release the exact same document. UBC / HR'S Linda Frost had fully supported BOTH versions, and from mid July 2008 was frantically setting up my THIRD Psychiatric Evaluation to be performed by UBC'S Dr. Alan Buchanan on June 04,2009.

 VERSION #1.

1.There cannot be TWO Versions of any Legal Document, it is 100% Clear that there is.

2. On a Legal Document in this case a Forensic Psychiatric Medical / Criminal Report. NO Error, NO Deletions or Additions are allowed by Law. Error is Evident (Explained Later). Deletions Evident in Version ONE. Additions not Evident but likely as we continue.

3. No signature.

4 Version  #1 for certain has been tampered with, so a violation of the Law is CONFIRMED. The First FORGERY.

VERSION # 1 is a proved FORGERY, used as TRUE by The CUPE Nationals Dianne Jolly, UBC / HR'S Lindi Frost, UBC Forensic Psychiatrist Dr. Elizabeth Zoffmann, The British Columbia College Of Physicians and Surgeons Dr. Robert Vroom, my past family Doctor (Name withheld for privacy reasoning.) Also UBC Psychiatrist Dr. Alan Buchanan who DENIED the existence of VERSION # 1, and THREATENED continued Psychiatric Evaluations to continue beyond 2009 if I continued to believe in its existence. 

By the end of 2009 UBC / HR'S Lindi Frost has now been promoted for her run on ME.  

 

UBC / HR'S Michael Vizolyi is now the LEAD.  From early 2010 into 2019 I pursue to get this FORGERY REMOVED from my Medical / Criminal Record.... IGNORED.

In early 2017 I had started my Employment as a Engineering Change Planner at a Vancouver Shipyard. A requirement for CONTINUED Employment was NATO Criminal Record Clearance.  I was on UBC / HR'S Vizolyis case to GET THIS REMOVED... IGNORED.

In 2017 I even approached British Columbia Government. 

In mid June 2019 I had to leave my Employment as I could NOT Complete the required for employment NATO Criminal Record Clearance.

In December 2019 I found a long searched for PHOTO ID of UBC Forensic Psychiatrist Dr. Elizabeth Zoffmann.

I CONCLUDED THAT I NEVER MET HER... THIS WAS A BRAND NEW TWIST TO A VERY VINDICTIVE STORY.

• TWO Differently Formatted Versions, with omissions and obvious additions, as in the assignment to have me committed under the mental health act, and the apology for failure. 
• The Date of Evaluation wrong, SUNDAY MARCH 25, 2001.
• My name spelled wrong 4 times... Mr. Steward which is not ME.
• From the 2001 Evaluation, there is content from 2005 / 2006. My belief written in the latter half of 2006, with an EVIL plot.
• Four times it repeats that weapons (FIRE ARMS)are suspected, to be proved by collateral means. (planned events yet to come beyond 2006). 
• It even identifies my attempted murder, if it is not a FORGERY.  
• After my THIRD Psychiatric Evaluation on June 04, 2009 I medically proved the FORGERY,  from the Differential Diagnosis Axis 1V and Axis V the GAF Score was MISSING from BOTH versions.

NO GAF SCORE ON EITHER VERSION = BOTH VERSIONS ARE FORGERIES... I cannot be Proved Wrong on that FORGERY CALL on TWO versions.  It May even get worse, with my December 2019 discovery. That I never even underwent a Forensic Psychiatric Evaluation, a fact that I was MADE believe by The CUPE National and UBC / HR. Now from December 2019 I am looking at the probably of an IMPOSTER impersonating a Forensic Psychiatrist. As in The President of The CUPE National.  
 

VERSION # 1 ISSUED AND USED AS TRUE BY THE CUPE NATIONALS Dianne Jolly on December 6 2006.

This was the Version that in August 2007 both Dr. Robert Vroom from The College Of Physicians and Surgeons and Dr. Elizabeth Zoffmann approved as 100% truth and authentic. This version was used by The CUPE National's Dianne Jolly to expel me from CUPE for NON Belief. This version was also used from July 2008 by UBC / HR'S Lindi Frost to take me to my THRID Psychiatric Evaluation performed by UBC Psychiatrist Dr. Alan Buchanan. This THIRD Psychiatric Evaluation was on June 04, 2009. Dr. Alan Buchanan would rule this version as NON Existent and my belief in Version 1 as Factor of my continued Mental Illness by me. Buchanan Threatens MORE Psychiatric evaluations on me to follow beyond 2009 if I continue to believe in the existence of a Version 1 BELOW. 👇.
